Services

Life Center provides in-home supports to children and adults with disabilities. These supports, identified in a coordinated effort by the family and the Supports Coordinator, are carried out by Life Center employees as a function of the individual’s Personal Plan. We provide training to our staff to assist them in not only a greater understanding of their role, but also to assure the opportunity to build relationships as bridges to a more successful and fulfilling life for the person supported. We believe through empowerment and choice, individuals with disabilities can lead more independent and meaningful lives.

Residential Services

In our residential settings, your loved ones will have the opportunity to create and sustain lasting friendships, while realizing their greatest potential. We provide all aspects of care to adults with disabilities in residential licensed and unlicensed settings. Our residential settings promote maximum intellectual and emotional growth through everyday activities and community integration. The group home staff are committed to giving each individual the opportunity to live the life they choose

In-Home Services

We offer In Home Supports to individuals and their families by providing Community Living Supports as identified in the Person Centered Plan. Individuals may live on their own, with a friend or peer, or even live with a family member. Our staff provides enrichment, training, and encouragement with tasks such as household duties, personal care needs, transportation, shopping, and errands. Our goal is to assist the individuals we support to feel safe, successful and empowered in the home and setting they have chosen.

Youth Mentoring

Our youth mentoring program provides children with support and assistance in establishing and maintaining meaningful relationships both at home and in the community. Our supports are focused entirely on minor children living in the home of a family member or legal guardian. Our mentors provide:

  • Role modeling
  • Skill building
  • Education
  • Supports with social acceptance and community inclusion
