Life Center provides in-home supports
to children and adults with disabilities. These supports,
identified by family and clinical staff, are carried
out by Life Center employees as a function of the individual’s
personal plan. We view our staff as mentors to those
we support. We provide comprehensive training to our
staff to assist them in not only a greater understanding
of their role, but also to assure the opportunity to
build relationships as a bridge to a more successful
and fulfilling life for the person supported. We believe
through empowerment and choice, individuals with disabilities
can lead a more independent and meaningful life.
Life Center offers lifestyle coaching and mentoring in
the areas of:
- Healthy living strategies
- Dietary supports
- Medication training and/or administration
- Individual money management
- Personal care
- Obtaining and/or maintaining benefits and entitlements
- Healthy relationships
- Conflict resolution
- Stress management
- Recreational and social activities
- Guidance with appropriate choices and decisions
- Developing and sustaining natural supports
- Achieving dreams
- Transportation to medical, employment and personal appointments
- Day services that offer skill enhancement to promote successful community
inclusion
